All you need to do is complete the standing order mandate and return it to us. As all the necessary records have been kept we know all your restoration details and nothing else is necessary unless you wish to upgrade your membership, to do this click here.

If you have not been provided with a statement form from a previous restoration, Simply contact us on 0113 2525428 or email us at Stephen@denlabcareplan.co.uk and inform us that you wish to become a member of Denlab Careplan. We will then contact your dentist and the laboratory that made your restoration to establish that they can provide the details so that the Denlab Careplan membership can provide you with the maintenance that is needed.
